- Dedicated to helping patients manage their disease.
- Patients will first be assessed via a 2-D echocardiography. They will be put on this programme if the 2-D results show that their hearts are not functioning well.
- Patients will go through a 6-minute walk session which will assess their exercise capacity.
- Patients are empowered to control their disease through knowledge on their condition and by keeping track of their progress.
